CleanImplant Community Membership – Terms & Conditions

This page sets out the terms and conditions of the Community Membership of the CleanImplant Foundation CIF GmbH – here „Cleanimplant“ – and should be read carefully. Any variations made to these Terms & Conditions may be made by the CleanImplant Foundation without express written notice to existing members and will be effective fourteen (14) days after the posting on the CleanImplant’s website.

The range of membership benefits current at the date of this application is set out at www.cleanimplant.org/become-a-member. The CleanImplant Foundation may suspend, vary or withdraw any of these services without notice. The website will be updated accordingly.

Your membership year

Your membership year starts on the date that you join the CleanImplant Community. The initial membership year lasts for a period of 12 months (Minimum Membership Period) and becomes due for renewal on the yearly anniversary of the Membership Start Date. Your membership will be continued unless a written cancellation (via letter or email) is received by CleanImplant or payment is not received. If no cancellation notice is received you move to a yearly rolling contract.

Minimum membership period

In order to allow CleanImplant offer a reliable service to members and to plan its resources, we require that all members maintain their membership for a minimum period of 12 months from joining. The Minimum Membership Period runs for 12 months from the Membership Start Date as defined above. After your Minimum Membership Period, you move onto a yearly rolling contract to have access to all benefits and privileges as a member.

Cancellation policy

You may cancel your membership by notifying CleanImplant, such notice to expire after the Minimum Membership Period has elapsed. CleanImplant requires at least one month’s notice for membership cancellations. The notice period will be calculated from the point of receipt of the cancellation instruction by CleanImplant, and the  Membership will end with the next anniversary of the Membership Start Date. Re-admission to membership must be within 2 months of resignation or termination or the membership will be considered a new member and may incur the first year membership fee. In case that the payment has not been received at the relevant anniversary, any outstanding balances must be paid upon cancellation.

We aim to acknowledge membership cancellations within one week of receipt. For this reason, if you have not received a response to your resignation within two weeks, it is essential that you contact CleanImplant to ensure that your letter or email has been received. Please note that if you cancel your payment instructions, this will not automatically cancel your membership. It is therefore important that you contact CleanImplant as soon as possible if you wish to resign from membership. If you decide to leave the CleanImplant Community, we would appreciate it if you would tell us why. We may ask you to complete a short survey, which will help us improve our services and better meet your needs in the future.

Termination of membership

If membership is terminated by expulsion no refund of membership subscriptions will be made and any balance due for the Minimum Membership Period shall remain due and payable. On cancellation taking effect or on other termination the benefits of the membership will cease and you will not be entitled to the benefit to be provided or fulfilled after the date of cancellation/termination.

PAYMENT

Subscription fees

Subscription fees for CleanImplant Community members are published on the CleanImplant Foundation website www.cleanimplant.org/become-a-member. The first year contribution varies from the contribution of the following membership years. The higher first-year-contribution reflects CleanImplant’s workload for the individual treatment and initial tasks for the new member. The membership fees will be subject to annual review and the website updated accordingly. Membership fees do not vary depending on the payment method. Possible changes to membership fees will only be made at the time of annual renewal of your membership.

Membership offers

From time to time CleanImplant may make special offers to encourage member recruitment. These offers may include incentives that vary the terms and conditions as laid out here (for example, by offering ‘13 months for the price of 12’) for those eligible for and who accept such offers (‘Eligible Acceptances’). These terms and conditions will be varied only as regards the Eligible Acceptances and only to the extent specified in the material accompanying the membership offer in question, all other aspects of these terms and conditions remain in force as regards all members (including Eligible Acceptances) unless otherwise notified.

Paying for your membership

You can pay online for your CleanImplant membership by credit card. Many members appreciate the convenience of Direct Bank Transfer. Please contact CleanImplant if you wish to do so. Changes to your payment method can be made only by contacting CleanImplant. Membership cannot be confirmed until payment is processed.

Claiming tax relief

CleanImplant Foundation CIF GmbH acts as a non-for-profit organization but is not a registered charity. Thus, a contribution to CleanImplant may be deducted as a business expense but not as a charitable contribution. If in any doubt, you should contact your tax advisor. When you first join the CleanImplant Community, or on the renewal of membership, you will be provided with a ‘Confirmation of payment or renewal’ invoice. Please keep this safe as it can be used as evidence when reclaiming your tax.

Unpaid subscription fees

CleanImplant is a ‘not-for-profit’ organization, and membership subscriptions form an important part of the support. Unpaid subscription fees, therefore, impact directly on our ability to plan our work and provide services for members. Consequently, CleanImplant reserves the right to take  the necessary steps to recover unpaid subscription fees, including legal action where necessary. In any circumstance where the subscription fees remain unpaid following the due date for payment, CleanImplant specifically reserves the following rights: a) to demand immediate payment of any outstanding sums due, b) to charge an administration fee of € 25 plus any costs of recovery, c) to add interest to any outstanding amounts at 4% above Euribor base rate.

If you anticipate any problems with the payment of your subscription, please contact CleanImplant at the earliest opportunity to discuss.  In particular, please ensure that you contact us if you intend to cancel payment instruction and/or use another Creditcard or form of payment, otherwise CleanImplant will eventually incur bank charges. You agree that any outstanding amounts on your membership account must be cleared on termination (and for the avoidance of doubt) before you can be re-admitted to CleanImplant Community Membership.

Refunds

On cancellation of membership, members who have paid in advance may consider that the notice period will be calculated from the point of receipt of the cancellation instruction by CleanImplant and the  Membership will end with the next anniversary of the Membership Start Date. There is no refund of any membership fees should you cancel your membership at any stage during your membership year.

 

RENEWING MEMBERSHIP

Renewal of your membership

Your membership will be continued and moved onto a yearly rolling contract unless a written cancellation (via letter or email) is received by CleanImplant. If you pay by Creditcard, fees will be taken at the new, reduced rate unless we receive notice of termination

Payment for renewed membership

An invoice will be sent to you before your membership renewal date which is payable prior to your renewal date for membership fees paid annually. If you pay your membership subscription by credit card, CleanImplant will charge your credit card with the membership fee for the forthcoming year. If you wish to change your payment method, it is essential that you advise CleanImplant immediately and make sure that your payment reaches CleanImplant at least one week before the Membership Anniversary, to ensure that your membership is not cancelled by us due to default.   

 

USING CLEANIMPLANT FOUNDATION MATERIAL

We are the owner of all intellectual property rights in our website, and in all the material, advice documents, templates published on it.  Those works are protected by copyright laws and treaties around the world.  All such rights are reserved. CleanImplant will provide all members a

–  personalized membership certificate 

–  optional membership listing

–  optional personalized implant-type certificate

–  CleanImplant membership-logo

you may use, copy or print in the purpose of personal and commercial purpose. You must not use any part of the materials on the CleanImplant website for commercial purposes, other than as set out above, without obtaining a written license to do so from us. If you breach these terms of use, your right to use our website will cease immediately and you must, at our option, return or destroy any copies of the materials you have made.

MEMBERSHIP PREFERENCES

How we communicate with you

When you join the CleanImplant Community, you provide us with your personal details, including your address and email. By giving us this information, you are agree that we may contact you from time to time (by email, SMS text message or post), with information related to services, products and events. You may, at any time, opt out of receiving communications from CleanImplant, change your contact details by contacting CleanImplant.

Privacy

We take your privacy very seriously. The information you provide on the registration form is „personal information“. This information is being collected for the purpose of processing your application, to provide you with membership services and to keep you informed of upcoming events. The intended recipients of this information will be only CleanImplant Foundation and its service providers. Please note that the provision of this information by you is voluntary. However, if you do not provide the information requested, CleanImplant may be unable to process your registration and provide all agreed services. You have the right to access to, and alteration of, personal information concerning yourself held by CleanImplant. 

With accepting the Membership registration you can opt-in to publish your contact data on the register of supporting CleanImplant members on the CleanImplant website to give orientation to interested patients. This service will start within the year 2019. If you decide to change your decision and opt out of this service, please contact CleanImplant. You must notify CleanImplant of changes to your contact and private information details in writing otherwise it may not be possible to make the services available to you. 

Complaints procedure

CleanImplant aims to provide all members with a high level of service and to deliver the benefits and privileges of membership as laid out in the Membership Definitions. However, there may be times when, for whatever reason, the service provided does not meet members’ expectations. Should this occur, in the first instance we encourage members to contact CleanImplant to discuss the matter and to give us the opportunity to investigate and assess the complaint.
